Video transparency: a powerful tool for patient safety and quality improvement

Transparency can be a powerful driver of better healthcare quality. In 2013, it was proposed that recording video data in healthcare begin with the several medical procedures that are already video based (cardiac stent placement, arthroscopic surgery, colonoscopy, etc). Different from the past era when recording required new hardware, most of the procedures performed in medicine today are now mediated by video, enabling a new opportunity for learning health systems. Some hospitals, like Johns Hopkins, have even built operating rooms equipped with cameras—some of these are used to monitor room turnover times for scheduling efficiency, while others have been installed in anticipation of a future of increased transparency.

Replicating changes in hand hygiene in a surgical intensive care unit with remote video auditing and feedback

Using remote video auditing (RVA) and real-time feedback, we replicated health care workers hand hygiene in a second intensive care unit. During the first 4 weeks using RVA without feedback, the compliance rate was 30.42%. The rate during the 64-week postfeedback period (initial 16 and 48 weeks maintenance) with RVA and feedback exceeded 80% on average. These data demonstrate that improved hand hygiene was achieved and sustained with the use of RVA and feedback

The Society of Gastroenterology Nurses and Associates (SGNA) outlines practice standards for manual cleaning of endoscopic retrograde cholangiopancreatography endoscopes before being placed into a automated endoscope reprocessors (AER) between patients to prevent the transmission of pathogens. Even though there are detailed guidelines for reprocessing endoscopes there have been multiple reports of patients acquiring CarbapenemResistant Enterobacteriaceae (CRE) following an endoscopic retrograde cholangiopancreatography (ERCP). Since pathogen transmission can be related to improper cleaning of the endoscope between patients this initiative was designed to assess adherence to the multi-step process for manual cleaning of ERCP endoscopes with the use of remote video auditing (RVA).

Using High-Technology to Enforce Low-Technology Safety Measures: The Use of Third-party Remote Video Auditing and Real-time Feedback in Healthcare

Hand hygiene is an important factor in protecting patients from healthcare-associated infections, yet reported compliance with hand hygiene averages about 38.7% with mean baseline rates ranging from 5% to 89%. Intensive care units report lower rates of hand hygiene compliance (30%–40%) compared with other healthcare settings (50%–60%). The majority of studies on hand hygiene are based on direct measurements, such as intermittent human observation, healthcare worker self-reporting, and indirect measurement of hand hygiene product usage .
